WORKERS' COMPENSATION

 

What is Workers' Compensation?

 

Workers' Compensation provides payment for medical expenses for employees who incur an injury on the job and must see an authorized physician or go to a medical facility.  It also pays a percentage of wages to an employee who has been placed off work by an authorized physician due to a workplace injury.  Full-time employees may supplement the remaining percentage of their wages with their accrued leave time.

 

Who is eligible for Workers' Compensation?

 

Any full-time or part-time (Adjunct, Student Assistant, Part-time hourly) employee of Valencia Community College is eligible for Workers' Compensation if s/he is injured while performing duties within the course and scope of his/her employment.

 

Who do I contact at the College?

 

Mariann Swenson, in Human Resources, extension 8028, provides daily assistance to employees and their departments. 

 

What types of forms are involved if I'm injured on the job?

 

1.                  College Incident Report: completed first by Security Services

2.                  FCCRMC Accident-Incident Form: completed by the injured employee and signed by the supervisor for any injury

3.                  First Report of Injury: completed only if you seek medical attention

4.                  Medical Referral Form: provided by Human Resources prior to your visit to an authorized physician; should be returned to Human Resources after the authorized physician completes the bottom portion

5.                  Certificate of Absence: completed for any time off work; Illness-In-Line-of-Duty leave must be accompanied by a doctor's statement authorizing the employee to be off of work

 

What are the step-by-step procedures that need to be followed once an injury occurs?

 

If you need immediate medical assistance, call 911 and the College's Security Services.

 

After you've contacted Security and completed the College Incident Report:

·        Report the injury to your supervisor, if you haven't already done so, and contact Human Resources at extension 8028 or extension 8033.

·        Complete a FCCRMC Accident-Incident Report and submit it to Human Resources.

 

IF you are seeking medical attention:

 

·        Complete a First Report of Injury form.

·        Obtain a Medical Referral form from Human Resources.

 

Do not go to your own personal physician as it will not be covered under Workers' Compensation.

 

·        Advise your supervisor and Human Resources of the outcome of your visit.

·        Send copies of the doctor's note to Human Resources and your supervisor.

 

 

 

Who are the authorized physicians?

 

Once you've received a Medical Referral form from Human Resources, you may go to any Centra Care facility.  If you are referred to a specialist, a Nurse Case Manager from the Corvel Corporation will make the necessary arrangements for the referral.  If you go to a physician who is not an authorized Workers' Compensation doctor, your expenses will not be covered.  Be aware, too, that group health insurance will not pay for expenses related to on-the-job injuries.  If you experience a life-threatening injury, call 911 for transport to the nearest emergency room at a local hospital. 

 

What do I do if my injury occurs after Human Resources is closed, and I need medical attention?

 

The College's Security Services is open 24 hours.  Once you've reported the injury to Security, go to the nearest Centra Care facility if you need to seek medical attention.  If Centra Care is closed, go to the nearest Florida Hospital emergency room.  In addition, contact Human Resources and advise your supervisor of the injury on the next business day or sooner, if possible, and complete the necessary paperwork.  If you do not need medical assistance at the time that the injury occurred, contact Security Services; and, then contact Human Resources and your supervisor on the next available business day.

 

What do I do after I've seen the doctor?

 

The Centra Care facility will complete the bottom portion of the Medical Referral form.  This will explain what you were seen for, if you need to be referred to a specialist, and if you can return to full duties at work.  It may also include the date for your follow-up visit if necessary.  This form is to be sent to Human Resources and a copy given to your supervisor.

 

What happens if I have to be off of work due to my injury?

 

The form you receive from Centra Care may also include the length of time and/or dates that a doctor has authorized you to be off of work.  The College pays for the first 12 days, known as Illness-In-Line-of-Duty Leave, that a full-time employee is off of work due to an on-the-job injury.  Afterwards, Workers' Compensation begins paying a percentage of the employee's wages until s/he is returned to work.
